What a Ride!!!! From Surgical Group of Arizona to Phoenix Unified Surgeons PLLC.

Our providers are busily working to get back on as many insurance plans as possible. If we are not on your insurance, be sure to let us know. Two of our providers, Dr. Cammarata and Dr. Fox-McClary are veterans, so we plan to get on Tricare and other military policies. Our providers see patients in Maricopa County and Yavapai County, so we want to get on all of the ACHHHS plans. Let us know if we are missing your insurance plans. If we place an application to your insurance plan, it can take up to six months or more for us to be credentialed as active providers on your plan. If we aren't on your insurance plan today, consider asking your insurance for a "Continuation of Care", which would allow us to take care of you even if we aren't on your plan. The insurance providers in this country are slow and often difficult to get on-plan with. Consider contacting your insurance plan and letting them know that you want Phoenix Unified Surgeons to be a preferred provider on your insurance plan. Together, you and Phoenix Unified Surgeons have the best chance of getting us access to your insurance plan. We are stronger together!

Hi there! Dr Shacket needs new cards for referrals. He also apologizes for missing your office opening, as I was in Washington having my GI scopes done on that particular day.
